BRIEF DESCRIPTION

The objective of this course is to empower participants from the PHC setting to contribute and join the process of PHC Transformation by learning the fundamentals and by developing a transformational project.

Learn new concepts from Health and Academic experts, based on scientific evidence.

“On Job” training is recommended.

SUMMARY

This Course has 3 Modules, 8 Sessions (to be organized according to the specific requests of the hospitals, either on place or online).

Module 1

• Session 1: PHC Context, Challenges and Strategy 
• Session 2: Transformation Priorities: Efficiency, Human Resources, Digitalization & Patient Engagement 
• Session 3: PHC transformation processes (DSRM + Agile Project Management) 

Module 2

• Session 4: Transformation1: Organizational Problem and Objectives for Transformation  
• Session 5: Transformation2: Transformation Design & Development and Demonstration  
• Session 6: Transformation3: Evaluation, Communication and Leadership for Action   

Module 3

• Session 7: Transformation Plan preparation 
• Session 8: Final Workshop. Course assessment.
